

A referral is required for all new patient appointments to ensure the patient is eligible for bariatric surgery. To be considered for weight-loss surgery, you must have one of the following:
- A body mass index (BMI) of 40 or higher
- A body mass index (BMI) between 35 and 40 plus an obesity-related condition, such as heart disease, diabetes, high blood pressure or severe sleep apnea
